MO-2012-203 Final Report - Fire on board Amaltal Columbia, 12 Sep ‘12 - Thursday, March 30, 2017
The fire was seated amongst bales of polypropylene bags stored in the fishmeal bagging room but the Commission was unable to establish with any certainty what started the fire. The Commission also found the ship’s design and systems for the prevention, detection, containment and fighting of a fire met the standards of the applicable maritime rules. AO-2014-006 Final Report - R44 mast-bump & in-flight break-up, Kahurangi Nat' Park, 7 Oct '14 - Thursday, March 30, 2017
The helicopter broke up in flight after one of the main rotor blades struck the cabin and the main rotor assembly separated from the rest of the helicopter as a result of “mast bumping.” It was very likely that the helicopter entered a low-G condition after crossing the Tasman Ranges downwind from Mt Arthur. Turbulence created by a strong southwest wind likely included severe downdraughts that prolonged the low-G condition which likely initiated the mast-bumping.
